Do I Need Both Inspections? All vehicles registered in New Hampshire must pass an annual safety inspection, and vehicles less than 20 years old must undergo an On-Board Diagnostic / emissions inspection in addition to the safety inspection.

The On-Board Diagnostic (OBD) system monitors the performance of light-duty passenger vehicles, including major components of the engine, such as the emission control components. The OBD alerts you when there is a defect that causes excess pollution. The best way to prepare for the emissions inspection is to keep your vehicle maintained throughout the year and per the guidelines in the owner's manual.
State inspection results are electronically transmitted to the Department of Transportation. Vehicles that pass both the NH safety inspection and emissions test will be granted an inspection sticker valid for the same time period as the registration. If a vehicle fails one or both inspections, it needs to be repaired and re-inspected.
